KHIE offers you the opportunity to request an accounting of disclosures (AoD) of your medical records in KHIE. This request may be made free of charge twice per year and will inform you which healthcare providers, if any, have accessed your medical records through KHIE. Upon receipt of your completed request, we will process your AoD report.
Please note, if you request an AoD on behalf of someone else, such as a minor child, you may be contacted regarding additional documentation required to fulfill your request.
If you would like to receive an email acknowledgement of receipt of your completed request for an AoD, please include your email address on the form.
The AoD report will be sent within 30 days of the receipt of your request. Please remember to check your spam/junk folder if you requested to receive your report via email.
