Make a Case for KHIE

A use case is a story describing how a KHIE service or services are used by Participants to accomplish specific healthcare goals.

KHIE Supports the Exchange of Health Information

Making health information secure and readily available from multiple healthcare providers into one aggregated patient chart when and where it is needed is the primary use case for KHIE, as use cases support the exchange of health information to demonstrate improved care coordination and health outcomes.

KHIE Supports Care Coordination

By accessing crucial information from KHIE, you have an opportunity to improve care coordination and improve efficiency of care for your patients,

Examples of Use Cases to improve Care Coordination

What are our Participants telling us about their Use Cases?

Event Notifications for Bariatric Patients

Ephraim McDowell Regional Medical Center leveraged KHIE to enhance patient outcomes and improve resource management. KHIE facilitated effective patient monitoring and transfer management, ensuring continuity of care and optimizing resource utilization amidst rapid patient turnover.

The KHIE Portal offered hospital clinicians an efficient way to gather information related to their bariatric patients from one source to inform postprocedure progress. The Event Notifications were set up to provide real-time updates (1) when a bariatric patient was admitted to another healthcare organization or (2) when a patient was treated for any complications related to the bariatric procedure.

Enhancing Pharmacy Care for Substance Use Patients

Med-Save Pharmacy incorporated KHIE into its daily workflow when serving outpatients in substance use programs, many of whom are Medicaid patients receiving care from a variety of healthcare providers in different locations. The lack of complete medical histories for these patients is effectively addressed through KHIE. It turns out that using this tool to serve patients also increases the pharmacy’s operational efficiency.

The KHIE Portal offers pharmacists an efficient way to gather contact information from other healthcare organizations. Accessing the Medicaid Claims data in KHIE provides a better picture of the patient’s health status, current treatment plan, and past care received; thus, minimizing the tedious process of information gathering.

Reconciling Medications for Med-Packaging Patients

Med-Save Pharmacy leveraged KHIE’s secure access to comprehensive health information to enhance its med-packaging service that supports individuals in assisted living and those requiring additional care at home. The data in the KHIE Portal and Event Notification Service emerged as valuable tools for the pharmacy. KHIE facilitates healthcare coordination by making the Medicaid Claims data available for pharmacy staff to manage medications for its Medicaid patients.

Event Notifications were set up to track patients as they are admitted to an emergency room or hospital, informing pharmacists when their patients move through the healthcare system.
