Taking Action on Patient's SDoH Needs
The topic of addressing social determinants of health (SDoH) plays a crucial role in patients' overall health outcomes. Understanding the social, environmental, and economic context in which patients live is critical to deliveirng comprehensive care. To continue the quest to improve health and reduce health disparities for Kentuckians, KHIE captures SDoH details from ICD-10 Z codes when present on shared continuity of care documents. This ensures all members of a patient's care team, regardless of setting, can be informed and act on the patient's social needs.
