CMS CoP Hospital Electronic Patient Event Notifications

KHIE can help your hospital meet the new CMS requirement for electronic Patient Event Notification Condition of Participation (CoP).  KHIE offers this service free to KHIE Participants.  

In a continued effort to support its Participants, KHIE is prepared to serve as an intermediary support for those hospitals seeking to meet the new CMS requirements for the CoP Hospital Electronic Patient Event Notifications.

KHIE is the logical choice for meeting your needs, as most hospitals in Kentucky are already participating with KHIE and contributing admit, discharge, and transfer (ADT) messages to the Health Information Exchange.  KHIE is in a unique position to meet the needs of both the 'sending' hospitals and the 'receiving' Primary Care Providers (PCPs) and the post-acute care providers (PACs) that the hospitals are required to notify.  

The final rule was effective May 1, 2021. With the intent to improve care coordination, the rule required hospitals (including psychiatric and critical access hospitals) with an EHR to send electronic patient event notifications making PCPs and PACS aware when patients are admitted, discharged, or transferred from a hospital's Inpatient Services or Emergency Department (ED). 

If you participate in Medicare or Medicaid and have an electronic system, this new notification requirement applies to you.  You must use reasonable efforts to send real-time notifications, directly or through an intermediary such as KHIE, to the patient's PACS and PCPs.​

​The Minimum ADT Content Required by KHIE to meet the Requirement

  • Patient Name
  • Treating Practitioner and NPI
  • Sending Hospital Name (your hospital)

  • Name of Recipient

What triggers sending a notification to a Recipient?
  • ED registration (Admit)
  • Transfer from the hospital's ED to the hospital's Inpatient Services
  • Discharge from the hospital's ED
  • Admission to the hospital's Inpatient Services
  • Discharge or Transfer from the hospital's Inpatient Services
Must be sent in real time
  • At the time of registration or admission
  • Immediately prior to or at the time of discharge or transfer
Who receives the notifications?
PACs = all applicable post acute care services providers and suppliers 
    • the PAC from which the patient was transferred to the hospital
    • the PAC to which the patient is being transferred or referred from the hospital
      • hospices, home health agencies, skilled nursing facilities
PCPs = a patient's established PCP practitioner or group or other practitioner/group identified by the patient as primarily responsible for the patient's care 
    • the PCP who has a care relationship that the patient recognizes as primary
    • the PCP who is evidenced by documentation of the relationship in the patient's medical record
    • the PCP who referred the patient to the hospital
      • examples of common PCPs include practitioners who specialize in general or family medicine, internal medicine, geriatrics or pediatrics
What does making 'reasonable efforts' mean?
Hospitals are required to use reasonable efforts to send ADT notifications to the patient's PACs and PCPs.  The general meaning of reasonable efforts implies 'that what can be done should be done' in the context of and purpose of a particular circumstance.  KHIE will work diligently to ensure compliance by supporting multiple channels for delivery of notifications and querying multiple provider directories.

Where's the Proof?
Although hospitals are not required to ensure receipt of the ADT notifications, KHIE will provide a monthly report that shows the number of notifications that were delivered and the number of those that were not delivered.
